Brazilian Butt Lift - BBL

The Brazilian Butt Lift involves a comprehensive removal of excess and undesirable fat from the entire body including the arms, abdomen, flanks, thighs, and back followed by the strategic transfer of the purified fat into the deflated buttocks. Newport Beach, Orange County Surgeon Our Surgical Team, routinely removes 1.5 to 2 Liters of fat which when purified allows for 200 to 400 cc of fat grafting per buttocks cheek. Our Surgical Team guides the injection of fat into preplanned buttocks zones in order to rejuvenate the buttocks aesthetics. Placement of fat over the upper and middle 1/3rd of the central buttock mound will result in improved fullness of the buttocks proper. Our Surgical Team utilizes several techniques to maximize fat graft survival. These include limiting the intensity of the negative pressure exposure on the fat cells during fat harvesting. Limiting the suction strength of the liposuction to approximately 10mm Hg avoids pressure damage to these fragile fat cells. In addition, the transplant time, i.e. interval during which the fat cells are out of the body, is minimized in order to avoid cellular compromise during the purification process. The fat cells are handled gently and expeditiously during this process in order to avoid mechanical injury as well as to minimize ischemic time. Specifically, the purification process involves washing of the fat cells with a gentamycin antibiotic solution not only to avoid infection but also to remove any unwanted residue and cell products, a liquid oily film, and unwanted aqueous plasma fluid. Following purification, these cells are compacted and injected into the buttocks mound using fine 1 to 3 mm cannulas. The distribution of fat cell injection is essential to achieving an aesthetically pleasing and natural result. Our Surgical Team preserves the buttocks aesthetics by routinely injecting the central upper 2/3rds of the buttocks cheek and avoiding the naturally occurring lateral cheek dimple. However, a comprehensive analysis of your buttocks will dictate the exact location of fat transfers. Another critical principle of fat grafting revolves around maximizing the grafted fat cell surface area to volume ratio in an attempt to re-establish blood supply to the transplanted and ischemic cells. Two maneuvers are used to ensure optimum graft take. They include the transfer of fat grafts in individual 0.5 to 2.0cc fat packs and layering the fat packs in multiple planes including the muscle, fascia, deep and superficial fat. A wide distribution of fat cells in small packs optimizes graft take and subsequent results.

Patients can expect minimal downtime, improvement in buttocks contour, and minimal to moderate lift of redundant and sagging buttocks skin. The lifting of the buttocks skin is achieved from filling the laxity of the buttocks skin envelope. The lifting effect following fat grafting is best appreciated following the transfer of fat to the upper regions of the buttocks cheek. Buttock Enhancement with Implant Augmentation

Patients who present desiring significant enlargement of their buttocks following moderate to severe deflation of their bottoms and/or whom do not possess adequate fat reserves, are ideal candidates for buttock implants. Buttock implants are made of semi-solid silicone and possess a textured surface. This semi-solid design of the implants makes them resilient to daily mechanical stresses induced on our bottoms. The textured surface ensures adherence of the implant to the soft tissues and stable positioning of the implant that prohibits migration and displacement of the implant.

Buttock implants are inserted using well hidden and limited length inter-gulteal incisions placed in a gentle arc to follow the natural curve of the buttocks cheek. Our Surgical Team carefully dissects identical and symmetric intramuscular pockets where the silicone implants are to be positioned and then inserts them using minimal touch technique. Placement of the implant in an intramuscular(gluteus maximus) pocket provides several advantages including an implant that is well cushioned so that its edges are not visible and not easily palpable as well as to minimize the risk of infection or implant rejection. Minimizing risk of infection is ensured by meticulous attention to sterile technique, non-traumatic surgical dissection, and multi-layered closure of the incision lines.

Side Effects

  1. Prolonged swelling up to 2 to 3 weeks is not uncommon due to the overfilling of fat that is performed to compensate for an expected 30 to 40 % of fat graft loss that is observed over the first year following the fat transfer. Buttocks implant augmentation patients will expect to swell for two weeks as well due to more extensive pocket dissection. Most patients will notice aesthetically pleasing results at 2 weeks following surgery and stable volumes of correction and filling at one month following surgery.
  2. Temporary bruising is often associated with fat grafting surgery due to the extent of tissue trauma that is created by fat harvesting and injection protocol. Bruising is minimized by compression dressings and ensuring that patients do not have any blood clotting disorders or taking any blood-thinning medications or supplements. Most bruising subsides within 1 to 2 weeks. Buttocks implant augmentation utilizes drains in order to minimize swelling.
  3. Temporary numbness is not uncommon and secondary to inadvertent injury to small sensory nerves which are traumatized by the mechanical action of fat harvesting or due to the undermining involved for the creation of the implant pocket. Fortunately, patients should expect a full return of sensation as these nerves even when traumatized will regenerate and re-establish sensation.
  4. Under correction of the desired contour is not uncommon for Brazilian buttocks to lift patients as they need to remember that 20 to 30% of volume loss is expected over the first year following surgery. Despite this expected volume loss, most patients are ecstatic with their postoperative observed volume improvements.

Risks

  1. Deep vein thrombosis (DVT) may occur in the legs immediately following surgery requiring general anesthesia. DVT refers to the clotting off of leg veins which may result in compromised blood flow return from the legs; a more critical consequence may develop from this clot if it is dislodged and travels to the lungs causing pulmonary emboli. Although rare, pulmonary emboli are the leading cause of death following surgery. Measures are taken intraoperatively to avoid such a complication.2)Local skin or fat infection is uncommon and associated with individual patients with poor healing tendencies secondary to medical illnesses. Diabetes, nutritional deficiencies, and smoking are to blame for local infections. Well-controlled sugar levels in diabetic patients, optimizing nutrition, and cessation of smoking are critical to avoiding infections.
  2. Skin tissue compromise leading to skin loss is rare following fat grafting surgery and is associated with the filling of scarred tissues due to increased degree of undermining and tunneling required to transfer fat cells. Smokers are urged to stop smoking for 4 weeks prior to and for 6 weeks following surgery in order to minimize skin tissue compromise.
  3. Fat emboli syndrome is a rare risk factor that can be life-threatening if not detected in the early postoperatively. This condition is caused by transferred fat cells entering major blood vessels and being dispursed and getting clogged in life-sustaining organs. Patients may present with a multitude of symptoms ranging from uncontrolled hypertension to chest pain, lightheadedness, confusion, and difficulty breathing. Patients require immediate admission to a hospital for supportive therapy with intravenous fluids and possible respiratory support until these fat cells self dissolve.
  4. Implant infection following Buttocks implant augmentation has been undeniably high with some early studies showing a double-digit risk of infection (up to 20%). However, Our Surgical Team’s modification of this procedure has led to not a single buttock implant infection that required implant removal to date.

Buttock Enhancement Recovery

The recovery period for fat grafting surgery of the body with a return to most daily activities ranges from several days to 1 week. In contrast, buttocks implant augmentation requires several precautions over the first two weeks including avoiding sitting on your buttocks proper as well as not sleeping on your back as well. Patients are asked to forego physical activities and exercise for approximately 2 weeks following fat grafting transfer and for 1 month following buttocks implant surgery. Finally, patients are asked to forego important social engagements for approximately 1 month until volumes of contour correction are stabilized or until your implants have settled into their pockets.

Duration of Results

Patients who have undergone fat grafting surgery can expect aesthetically pleasing results for life. Patients will observe 30 to 40% of volume loss over the first year following surgery. Volume corrections one year following surgery should be considered stable. In contrast, your enlarged buttocks contour following buttocks implant augmentation is not prone to volume loss.

FREQUENTLY ASKED QUESTIONS

You may be a good candidate for buttock augmentation surgery if you have a flat bottom in jeans/clothes or if you have lost your back/buttocks or lateral buttock silhouette. When observed from the side, patients may notice replacement of the back buttock natural silhouette involving the gentle back curve of your back onto the buttocks with a more flattened silhouette. On front view, the patient may notice replacement of the lateral buttocks curve with a less desirable double hump appearance. Increasing flank and lateral thigh prominence following excess weight gain and concurrent loss of buttocks projection creates the undesirable changes and increasing buttocks droop. Individuals who have drooping of the buttocks region and flattening of the buttock cheeks due to aging can benefit from implant augmentation surgery Patients must be in good general health, both physically and emotionally, in order to pursue buttock augmentation surgery.

To perform buttock augmentation surgery, parallel incisions are made along the intergluteal crease (otherwise known as buttocks crack) and which extended superiorly and laterally following the natural curve of the buttocks mound. This incision is dissected down to the gluteus muscle, which is released over its superficial fibers to allow for creation of symmetric pockets within the muscle. A semi-solid silicone implant is then inserted into each pocket. The silicone implant is fit tightly in order to prevent implant migration. The dissected incision is then repaired in layers and includes the superficial fibers of muscle previously released as well as the soft tissue and the buttocks skin. Prior to closure, a large drain is placed and secured in order to remove any postoperative fluid accumulation. Finally, an expanded buttocks tape is applied to the incision lines and maintained for two weeks in order to minimize shear stresses on the implant and surgical incision lines.

On your initial visit, your surgeon will evaluate your health status by obtaining blood work and requesting a preoperative clearance from your primary care physician. Patients with underlying medical conditions will have to optimize their health status prior to proceeding. Patients with difficulty breathing or underlying pulmonary conditions, may not be found suitable for this surgery as it requires prolonged duration of lying on your stomach. In addition, smokers will be asked to quit smoking for 4 weeks prior and 6 weeks following surgery. Finally, you will need to arrange for a friend or a spouse to care for any small children who may need to be lifted since you will be sore for the first couple days following surgery.

Since your surgery will require general anesthesia, you must take several precautions prior to surgery. First, you will have to avoid eating and drinking after midnight on the day before surgery. This precaution ensures that your stomach is clear of digested foods that could potentially be aspirated into your lungs during induction of anesthesia. Since, you will have anesthetic medications administered throughout the case, you will remain groggy for several hours and will require a ride to and from the surgery center. You should choose a caretaker who is conscientious and who can spend the first night with you. As you will have to avoid sitting upright for the first two weeks, you should also ensure that your ride has a generous back seat that you can lay on your stomach for the ride home.

Buttocks augmentation surgery will lead to enhancement of the buttocks shape and improved projection of the buttocks contour. Patients are usually extremely satisfied with their immediate results and enjoy a lifetime of aesthetically balanced lower back to buttocks silhouette. Buttocks augmentation surgery can be performed at an office-based surgical facility, outpatient surgical facility, or at a hospital operating room. Since your surgery will require intra-muscular dissection and placement on your stomach for a prolonged period, general anesthesia will be required.

Patients can usually resume light activity several days following surgery. Patients may experience numbness of the buttocks skin overlying the implant that disappears over the first few days after surgery. Pain medication and antibiotics will be prescribed by your surgeon. Patients will wear both a specialized tape and garment over their buttocks region to secure the placement of the silicone implant during the swelling phase immediately following surgery. Patients may find most constraining the inability to sit upright for the first two weeks following surgery. In addition, patients are encouraged to sleep on their stomach for the first two weeks following surgery. Patients are allowed full physical activity and wearing of routine undergarment four weeks following surgery.

Silicone buttocks implants have proven to be safe, predictable, and maintain a low-risk of complications. There have not been reported cases of allergic reactions involving silicone buttock implants.
